About the project

The D-PACT: Dementia PersonAlised Care Team (D-PACT) was a research programme funded by the National Institute for Health and Care Research (PGfAR Award ID: RP-PG-0217-20004). 
Launched in November 2018, Phase 1 of this programme developed a dementia support worker intervention aimed at people with moderate to severe dementia and their family carers. The dementia support worker was based in primary care. Phase 2 evaluated this intervention in GP practices in a range of settings in Southwest and Northwest England.
Grounded in lived experience and co‑produced with practitioners, researchers and people affected by dementia, D‑PACT aims to improve continuity, coordination and personalisation of care across services.

About the event

This final event follows the first event held in the second half of 2024. Since then we have completed our final report (to be published shortly), the NHS 10-year plan has been launched and the health and social care landscape continues to evolve.
This final event, hosted by the Â鶹´«Ã½ and supported by the , marks the culmination of the project and is designed not only to share learning, but to actively explore how D‑PACT can be taken forward in real‑world settings.

This online event is structured across two linked sessions, to be held one week apart. This deliberate format is designed to offer flexibility and space for reflection, discussion and future-focused planning.
We hope that this event will be attended by those living with dementia, their family carers and supporters, health and social care professionals, service managers and commissioners, policy makers, researchers and academics or voluntary, community and third‑sector organisations.
Week 1
Friday 20 February 2026, 12:00–13.45: Sharing the learning (with optional discussion)
Week 1 will open with a series of short, focused talks including from a person living with dementia. Together, these talks will reflect on what D-PACT set out to achieve and what we learnt as well as what this might mean for the wider policy context, post-diagnostic services and those who receive the support.
After a short break, attendees are invited (optional) to return for a facilitated discussion. This session will provide space to ask in-depth questions, reflect on how D‑PACT might fit within individual services, organisations or systems and begin thinking about possible next steps in practice, research or commissioning
The session will be recorded, allowing participants (and colleagues who were unable to attend) to engage with the content at a time that suits them.

Week 2
Friday 27 February 2026, 12:00–13:00 Open-door discussion and next steps
One week later, we will host a one‑hour, optional open‑door online session. This informal session is particularly aimed at those who listened to the recording after Week 1, or wanted more time to reflect and/or to bring ideas, questions or proposals for applying D‑PACT in their own context. Insights from these discussions may help inform the development of a policy‑focused output following the event.
