The
D-PACT: Dementia PersonAlised Care Team
(D-PACT) was a research programme funded by the National Institute for Health and Care Research (PGfAR Award ID: RP-PG-0217-20004).
Launched in November 2018, Phase 1 of this programme developed a dementia support worker intervention aimed at people with moderate to severe dementia and their family carers. The dementia support worker was based in primary care. Phase 2 evaluated this intervention in GP practices in a range of settings in Southwest and Northwest England.
Grounded in lived experience and co‑produced with practitioners, researchers and people affected by dementia, D‑PACT aims to improve continuity, coordination and personalisation of care across services.
About the event
This final event follows the first event held in the second half of 2024. Since then we have completed our final report (to be published shortly), the NHS 10-year plan has been launched and the health and social care landscape continues to evolve.
This final event, hosted by the Âé¶¹´«Ã½ and supported by the , marks the culmination of the project and is designed not only to share learning, but to actively explore how D‑PACT can be taken forward in real‑world settings.
